This assignment (essay) is based on interview performed by student nurse on a person about how their chronic condition impacts their ADLs (RLT MODEL) and quality of life then Collaborate with them to establish Smart Care Goalsfor their management of their condition. Care priorities that they see for themselves and sits within the impact of their disease on them. Things they might want to change. What are things that they might want to change in their circumstances (SMART). A kind of motivational interview would be good.
The goals set for this client must be smart i.e specific, measurable ,attainable, relevant and time- based eg : pain management. You cant set goals that is not achievable for the client.
Student nurse should link underlying knowledge/ research of the pathophysiology into the written discussion using CRC to make sense of symptoms/situation BUT the focus is on the overall impacts to the individuals life.
