· Describe the assessment tool that you have chosen.
The assessment tool that I have chosen is the DSM-5. According to Kirst-Ashman and Hull (2017), “The DSM emphasizes characteristics of the various mental disorders without specifying causes; this process allows professionals to maintain their own theories regarding why such mental illnesses occur.” (p 194) This assessment tool not only can assist in diagnosing a mental health condition but also a substance use disorder.
· Explain why you would use this tool with a client.
The population of folks that I would like to serve in my social work practice are those who struggle with mental health conditions and/or substance use disorder. I would use the DSM-5 with a client that may be exhibiting a mental health disorder or possible substance use disorder. “A second benefit of the DSM is its helpfulness in evaluating and treating clients with mental disorders.” (Kirst-Ashman & Hull, 2017, p 194) It is important to get to the cause of a person’s behavior and the DSM is an important tool in helping with that.
· Explain how this tool could assist you in the client assessment process.
The DSM would be helpful when diving a little deeper into a client and their behavior. There are other one page assessments that provide a simple explanation or cause for behavior (for example the PHQ-9 for someone who may be experiencing depression) but in contrast the DSM provides a pretty solid tool for examining mental health disorders and substance use disorder in greater length.

would use a self-assessment template to gather information about the client’s needs and situation. Assessments are crucial in understanding needs and providing appropriate care and intervention (National Institute for Health and Care Excellence, 2022). Self-assessment is an assessment tool used in social work whereby the client is provided with a template to fill in information that would help the practitioner provide effective services to the client. I would choose this assessment tool because it enables the client to reassess their life by identifying their strengths and challenging areas in their life. It is a great tool that helps clients put their problems into perspective. While filling in the self-assessment template, the client can have time to think through every aspect of their life and categorize as needed in the template or form. Self-assessments are also great because they help a client recognize their strengths. This is important because many clients fail to recognize their strengths during challenging situations. By identifying personal strengths, it would be easier to formulate an intervention plan to help the client because one can use one strength to handle the challenges.
Client self-assessment would assist me in the assessment process by helping me gather and analyze crucial information needed to help the patient. The client would provide essential details such as personal responsibilities, needs, interests, goals and priorities, and health. It would help the client reflect on his life and capture what is happening in a few words. This information would guide me in helping the client by providing appropriate interventions. Providing appropriate intervention without comprehensive information about the client would be impossible. This assessment tool helps in doing that because it allows the client to provide as many details as possible that would help me provide effective services.
